A 12 year old child with diplegic idiopathic cerebral palsy.  

Clinical Details

Physical examination

Hip abduction (knee straight):        left   40°        right  40°

Hip abduction (knee 90°):               left 40 °         right  50°

rectus sign                               left   +           right  +

femoral anteversion                             left   75°        right  50°

tibial torsion                                      left 10°          right 5°

hip flexion (knee extended)                 left   45°         right  45°

Manual muscle test (MRC grading)

Hip abductors:        left  4 +       right  2 +

Knee extensors:       left  5        right  5

• What's the reason for the varus feet (especially left)? 
• Are the foot abnormalities caused by the transverse plane deformities, or vice versa? 
• Are these any of these deformities compromising function? 
• Would you do anything about them, and if so what would you do?
